It could be a food allergy. What kind of food is he eating? The three biggest allergens in food are corn, wheat, and soy, so make sure there is none of that in the food. It could be other ingredients as well, but those three are a good starting point.
It could also be yeast. Maddie has issues with yeast in her ears, in between her toes, and on her chin. I have to clean out her ears at least once a week, and for her paws and chin I use baby wipes with a solution of 1/2 white vinegar, and 1/2 rubbing alcohol. That usually takes care of her flare ups.
I hope Sunomi does not have food allergies, because if it's not those three ingredients it may be tough to figure out what he is allergic to. May have to do an elimination diet to find out.
I would try the vinegar and alcohol wipes and see if that helps him. Just make sure not to rub that on any open sores because it will burn.
